Adverse events in the healthcare system often have severe consequences for the patient and healthcare institutions. Thus, various risk management tools, such as Critical Incident Reporting Systems or surgical safety checklists, have recently been introduced to 11 Austrian healthcare centers to increase patient safety. However, our study shows that these instruments are not effective in all of the centers investigated. The effective implementation of these tools requires change management in the background to develop a lively patient safety culture. To facilitate this process, it is essential in this study to gain a deeper insight into the underlying implementation processes and problem-oriented perspectives. To investigate this issue, a qualitative approach was used to examine the current status of patient safety culture in healthcare centers.
